Customize the open ended question

The open ended question is asked after someone clicks the submit my answers button. The 3 survey questions will be recorded even if the person chooses to not answer the open ended question.

1. To customize this, head over to the health survey settings area:

2. Click on the sub-navigation tab labeled "Thank You Page"

3. Be sure the optional question is toggled on, then customize the current message and hit the save button at the bottom.
